Wall School District Enrollment Information Form

The Wall School District requires all families to complete an Enrollment Information Form for each student attending our schools.

Families may download the Enrollment Information form using the link below or pick up a copy at the school office. This form collects important details, including student demographics, contact information, family/guardian information, medical needs, emergency contacts, and language background. This information helps the school provide a safe, supportive, and well-informed educational environment for every student.

Please note:

  • A separate form must be completed for each student.
  • It is important that all information is filled out accurately and updated as needed.
  • The form includes optional permissions related to FERPA, military outreach, and college/university communications.

Completed forms should be returned to the school office prior to the start of the school year or upon enrollment.
